Follow the steps in the bottom left panel. Pay attention to the ones that "needs attentions" status.

Step One: Insert the macro
In a new or existng page, you can use the / or + method to insert the Form Hub macro.
click, search, select.
Step 2: Form select
From left hand panel, select 1 or 2 for prepackage templates. Option 3 let's create your own or choose from previously created ones.
Then select Open in Form Designer.
Step 4: Design
Use the field tool bar to insert fields into the middle canvas. Most config happens by clicking the field object. The right panel shows the configurations set in the selected field.
Step 5: Branding
Design or using saved branding kits. Upload banners, icons. Change fonts, background colors.
click Save the form.
Viewing results
The form defaults to having 3 tabs, Form, Responses: table view of the fields for each submission, Reports (analysis of the results)
